Principles

  • We will have one Integrated Service which  “Follows the Client Journey”
  • Single client service role will help drive synergies, bring clarity to costs and outputs of what we deliver providing a strong basis for strong funding negotiations.
  • Our service delivery is consistent and flexible supported by a Key Contact Person.
  • Our clients “express delight” with the service they receive at every stage of their journey.

Rationale


Drawing all aspects of direct client contact services involved in supporting our clients independence and lifestyle together under one team is designed to enhance the development of:

  • One integrated service that follows our client through their journey
  • Service options aligned to meet our clients life cycle and current clinical and independence needs
  • Service synergies across current service models Identified.
  • Reduced duplication of effort for our clients and staff
  • A Key Contact role that guides the client through our service options
  • A “pick & mix” approach to service options that might focus on an Independent Living, Peer support, Employment and Cultural focus
  • Together with the “gentle door opening approach” of Stakeholder Relations this role will provide a strong structured approach to successful future funding negotiations.


This is large service that will require leadership capable of bringing about service change with a client focus and expertise in integrating services.
